Contact us if you have any questions about health care and health care insurance coverage in Alberta.
Beginning April 1, 2009, Albertans can register for Alberta Health Care Insurance Plan (AHCIP) coverage and update their information at one of 20 registry locations across the province. Albertans wishing to register for health care or update their personal registration information may do so at one of the registry service centres.
Please note that payments can no longer be made in person. Payments may be made by mail, or through your financial institution. Please refer to the back of your premium statement for more information on payment options.
Name and address change forms (Notice of Change, form AHC0107) are available at any Alberta Registry Agent. Completed forms can be left with the registry agent who will forward it to AHCIP free of charge. Alternatively, please go to our forms page, open, print and complete the Notice of Change form and send to AHCIP directly.

Protect your health number
It is important for all Albertans to take steps to safeguard their personal health number and card. Please protect your Alberta Personal Health number and card by ensuring they are used only when you are obtaining publicly funded health services.
If you have any information about suspected or confirmed abuse of personal health numbers or cards, please call our Tip-Line at 1-866-278-5104*. In accordance with privacy legislation, any information reported is considered confidential. Tip-Line staff will not record any identifying information about the caller if a caller wishes to remain anonymous.
*Please note: The Tip-Line is only for calls relating to suspected/confirmed abuse of personal health numbers and/or cards. All other inquires regarding Alberta Health Care Insurance Plan registration, coverage and claims should be directed to our main office as noted above.
