Form instructions – Health Care Insurance Plan

New and revised forms and brochures are posted on the form pages on a regular basis. To avoid using an outdated form, we recommend you do not save any form. If you need a form, check the Forms pages to ensure you have the most current version of the form.

Alberta Health Care Insurance Plan (AHCIP) and Alberta Blue Cross forms on this site may be viewed and printed. Some forms may be filled in on the screen before printing.

Viewing your form

Forms are provided in portable document format (PDF). To view these files, you will need a recent version of Adobe Acrobat Reader installed on your computer. The latest version of the software is available for free from Adobe.

Filling in your form

If the form has fields that can be filled in on screen:

  1. The form will open with the pointer in the start position. Begin typing your information.
  2. Use the TAB key on your keyboard to move to the next box. You can use SHIFT + TAB to move back.
  3. Click your mouse button to fill in checkboxes.
  4. You can use the RESET FORM button on the form if you need to start over.
  5. Use the SAVE button on the form to save the form to your harddrive if you wish to complete it at a later date.

Printing and submitting your form

  • If the form cannot be filled in online, print off the form and fill in by hand using a pen.
  • Once you have completed the form online, print a copy using the PRINT button on the form or the print command on your computer.
  • Sign in all the appropriate areas and ensure all supporting documentation is attached. Submit the form either through mail or fax.
